Drive/Ride Permission Slip - Make sure your athletes are cleared to get to practice!
Field Trip Planner - Taking your team on a trip? Submit this form to the Main Office to acquire school permission
Van Driver's Information Form - Complete this form prior to driving an OLSH vehicle. Only needs to be completed once per school year.
Van Sign Out Form - Use this Google Sheet to sign out any of the school vans to transport students to your activity
Business Office Procedures for Athletics - Learn correct procedures to follow when running a fundraiser, requesting funds, and more
Athletic Money Submission Form - Submit this form to the Business Office along with any funds to be deposited into team account
Athletic Check Request Form - Submit this form to the Business Office when requesting reimbursement or a check to pay for team merch, supplies, registration fees, etc.
Fundraiser Request Form - Teams must submit this online form to the Advancement Office to have fundraisers approved
Cell Phone Communication Form - Need to communicate with your athletes via call/text? Complete this form to acquire parental and school permission.
Sport End of Season Forms - Submit these forms to the Main Office at the conclusion of your season for the school records and yearbook

DGM Photography - Schedule your team photos (Teams may choose their own photographer, but we have worked with DGM in the past); Coaches must send head shots and team photo to cmcdevitt@olsh.org for use on the athletics site and in the yearbook.
