Please download and complete the following physical form(s) in order to participate in your sport:

Sports Physical Packet INITIAL - Must be completed to participate in any OLSH sport

Section 7 Sports Physical Form - To be completed if you already have a physical on file but wish to participate in another sport during a different season

Section 8 Sports Physical Form - To be completed by a licensed Physician of Medicine or Osteopathic Medicine, clearing an injured student athlete for participation in their sport
